So we do treat women that’s right. I wanted to kind of talk about that.

Speaker 2: 

You know what I thought we would do? I thought we would just do some random questions from you because you must be like wondering about what you identify as one of the things

Speaker 3: 

I do today, I’m slew it tomorrow.

Speaker 2: 

Uh, and I thought we would, uh, you know, as somebody who is really the, uh, the driving force and the heart of this practice in terms of how we face our patients and how we face our employees, which is very important, you must have questions all the time about what the heck we’re doing around here.

Speaker 3: 

I mean, I have to admit, I started hormones with us or hormone therapy with our Leonor Brown. So I feel amazing, but I’m always curious. And women are in my age group, my friends, we chat about our libido. What happened to the libido. It used to be all over the place. And now it’s just kind of diminishing as we age. How do we reverse that? What steps do we start taking? And how do you treat the patient when they present with

Speaker 2: 

That’s a great question. I used to have the slide in my slide deck in which you had a box with a hundred different buttons and that was called women and then a box with only one button. And that was called a man. You know, and we used to say, this is the difference between a man and woman when it comes to libido. There’s so many different things that interplay. So first I’ll just talk about kind of hormones and hormone replacement in the, in the 45 to 55 year old woman. I think a lot of confusion really happens. This is a great example of how you as a patient are subject to the biases training education of your doctor. So often if your doctor didn’t learn her hormones or your doctor had an aunt with breast cancer who they attribute to hormones, or you, you’re not going to be offered hormones.

Speaker 2: 

And so you’re going to have to go seek out your own care when the data is quite clear, if you start hormone replacement therapy within 10 years of going through menopause, your risk of heart disease, breast cancer, and Alzheimer’s goes down, it’s a lifesaving or a lifestyle improving treatment. It’s not just meant to be some wacky thing that you’re doing just to kind of recapture you it’s really has a real health benefit, even more so for women than men, believe it or not really. And so really all you’re trying to do is maintain those in your body. Your hormones are little bitty, chemical activators that work all throughout your body and estrogen and a woman and testosterone women affects their skin, their hair, their bones, their breast tissue, their vaginal tissue, their heart, their lungs, everything, and their brain to ignore the importance of hormones in that age group, I think is a real shame.

A lot of times women will go seek out care and then they’ll end up in a very fancy, very, very expensive hormone replacement clinic. When in fact all these things are covered by insurance. They’re super easy for us to do, and then we can integrate it into a program to make you feel better through nutrition and supplements, and really help you up. Hormones are a very big driver of libido getting to your original question, right? But libido is affected by relationship issues, right? We talk about all the time that maybe, maybe a boyfriend will help your libido. You know, uh, it, it has helped them maybe if the towels or pick up his socks, you know, so there’s, there’s some interpersonal and relationship issues. There’s hormonal issues that can affect both your brain and how you consider and think about sex, but also hormonal issues and how your body responds to sex or to sexual impulses.

Speaker 2: 

If you’re not getting turned on anymore, if you’re not getting a clitoral erection, if you’re not getting lubricated, it’s going to be very hard for your libido to be high. It’s that simple. And if it hurts to have sex, your libido is probably going to be low. We consider the sexual response cycle to be what we call a circular and interrelated cycle. So your body, your mind, your hormones, neurologically, how you’re feeling has to be all intact. And then of course the psychology. And that’s why you can’t have one doctor that does all this. You have to have our pelvic floor physical therapist that helps with pelvic pain. You need hormones, maybe systemically hormones, vaginally, and then our sex therapist to help with those things that kind of come about later. But I can’t emphasize enough the importance of having

Habit. If you get out of the habit of having sex, the habit, then you’re going to get out of the habit of having sex. That’s important to make sure my husband,

Speaker 2: 

So there’s no, there’s, it’s very difficult to get a 60 year old woman who hasn’t had sex in 20 years to really want to and get in the habit of having sex. Again, I’m not 60, by the way, I understand. I’m not even saying that. I’m just saying that, you know, if your libido is diminishing, then number one, let’s make sure that your husband’s not your case. Just one’s husband is functioning well sexually, and that’s not, they’re not playing off each other because if you don’t want it and he can’t do it, then you guys aren’t going to do it anymore. If your libido starts diminishing, let’s make sure that the, you know, the playing field is ready. You know, make sure that you’re well lubricated, that nothing is hurting. That’s something we can do for you. We can make sure that you’re hormonally intact, but the way that testosterone estrogen work it’s about that first spark and how your brain first responds to the idea of sexual stimulus.

Speaker 2: 

If you, if you’re hormonally intact, you’re more likely to be positively influenced to try to have a sexual, intimate, uh, interaction. But if these things aren’t there, you may be averse to it. And so that’s where hormones help. It’s not going to get you across the finish line, but maybe it’ll get you started in the race. And then we have a number of medications we have by Lacey Addie, all these great medicines, right? People have always been looking for the female Viagra, guess what? We have that too. It’s called Viagra. It works for women as well. You just have to match up your problem with art therapy. And if you’re a woman out there with low libido, whether you’re 20 or 40 or 60 or 80, we’ve probably have something that’s going to be safe and effective for you to help you out. A lot of 20 year old women have a low libido from chronic use of oral contraceptives.

Speaker 2: 

We treat have to treat those women differently and we have these treatments they’re safe and effective. And it’s absolutely the kind of thing that you may not imagine to get advice from a men’s health show or a urologist. And you may think you’re OB-GYN may be the right place to go, but believe it or not, they weren’t trained in it any more than your primary care doctor was. But as urologists, we are sexual medicine doctors. This is exactly the place that you need to go to get that kind of information and advice. That was the best commercial ever for a woman with low libido. That’s right. She’s listening. The saddest commercial ever.
