Hi. I remember the first time you called me and you left your voicemail on my machine or thing at work. At my other job.
Dr. Mistry:
You didn’t believe it.
Donna Lee:
I didn’t. I said, “Who is this Sunny Mistry guy calling?”

Who’s this?
Donna Lee:
Who is this smooth talking dude calling me with a fake name?
Dr. Mistry:
Oh man.
Donna Lee:
Now we work together.
Dr. Mistry:
Donna Lee, you are the office manager of our urology practice. I am a board certified urology practice. We have been, we established ourselves in 2007, and we’re much bigger than just a little mom and pop, aren’t we?
Donna Lee:
Ugh, we’re growing.
Dr. Mistry:
We are the second largest urology group in town.
Donna Lee:
We have lots and lots of staff now.
Dr. Mistry:
We do. We do have lots of staff. Why don’t you tell people who our doctors are?
Donna Lee:
Our doctors are you, Dr. Mistry? M-I-S-T-R-Y, when you go to Google him. We also have our partners, Dr. Stacy Ong, Dr. Christopher Yang, and now we have a new one, Dr. Lucas Jacomides.
Dr. Mistry:
We were so happy that Dr. Jacomides joined us. He’s an exceptional, well-established member of the community. He was a Scott and White doctor for many years and just a funny and lighthearted…
Donna Lee:
Super nice.
Dr. Mistry:
…and you know what’s great about him?
Donna Lee:
That he fits right in with his dirty sense of humor?
Dr. Mistry:
That’s right. And he can see new patients right away.
Donna Lee:
That’s right.
Dr. Mistry:
So if you have a urologic condition, if you developed a kidney stone, you know, this last week and you need to see somebody, if you need a second opinion on your prostate cancer, or have blood in your urine, or erectile dysfunction, or low testosterone and need to be seen, we really pride ourselves on getting patients in right away, because I think that’s one of the most important things that we can do as a specialists for you is to take care of your problems.
Donna Lee:
Right?
Dr. Mistry:
Because we’re the ones presumably that know how to do it best.
Donna Lee:
And there’s just such a need for urology. I mean, holy moly, every time I go to a private, primary care group, they’re just begging for help in urology. So…
Dr. Mistry:
Well, men aren’t getting any younger in this country.
Donna Lee:
That’s true. And they’re certainly not taking care of themselves.
Dr. Mistry:
That’s right. And so whatever we can do to help…
Donna Lee:
That’s why we’re here.

And you know, that’s no joke. We have patients that listen to the show from all over the country. We even have patients that are coming to see us for vasectomies and for prostate cancer treatment. And we had one that actually traveled from Oklahoma for a kidney stone.
Donna Lee:
Oh wow. That’s a lot of traveling for a kidney stone. You think he would have passed it by the time he got here? Right.
Dr. Mistry:
Maybe. I’m glad he made it all the way here.
Donna Lee:
Speaking of vasectomies, it is kind of vasectomy season. I know a lot of urology groups like to have their patients get the March Madness fever and then they want to stay home and watch basketball all weekend, and they have their vasectomies.
Dr. Mistry:
I’ll tell you a funny story about that is last year, we tried to do a little promotion on Facebook called “Vas-madness,” and then I was sent a cease and desist order by the big guys in town…
Donna Lee:
Oh. I remember.
Dr. Mistry:
…telling me that they’ve…
Donna Lee:
They had that.
Dr. Mistry:
…had the trademark on it. I was like, there’s no way that a trademark on it. And then I went to the state and I looked it up and they really the trademark.
Donna Lee:
That’s why we renamed it.
Dr. Mistry:
That’s right.
Donna Lee:
The “Vasectomathon.”
Dr. Mistry:
The “Vasectomathon.” You can’t take that one from us.
Donna Lee:
That’s right. We need to trademark that.
Dr. Mistry:
You can’t take that one from us.
Donna Lee:
But it sure is a mouth-full.
Dr. Mistry:
The Superbowl of vasectomies. The “Vasectomathon.”
Donna Lee:
The Vasectomathon is just hard to say, but anyway, speaking to vasectomies…
Dr. Mistry:
But I think that we should trademark “carnival,” that way we can do vasectomy carnival, prostate carnival, kidney stone carnival.
Donna Lee:
You know, we’re going to get a complaint.
Dr. Mistry:
…even erectile dysfunction carnival.
Donna Lee:
People didn’t like “Make America Pee Again,” or they absolutely loved it when we had the event, but “prostate carnival,” people are going to get mad that, we’ll have to get clowns involved.
Dr. Mistry:
Man, prostate clowns. Go with the question.
Donna Lee:
We have a vasectomy question. “Dr. Mistry, I’m a 33 year old man. I have 3 children. I am tired of using condoms. My wife’s OB-GYN said she should not be on hormones for very much longer for her safety and health. And she yelled at me when I suggested that she get her tubes tied. How can you help with your special vasectomy?”
Dr. Mistry:
That’s a great question. You know, recently there has been scientific literature that was released that suggested that longterm use of oral contraceptives simply for contraception may not be in the best health interests of women. And so this kind of underscores both the benefit to you as a man making your own decisions about family planning, as well as the health of you as a couple by considering a vasectomy. Simply put, the vasectomy is a simple procedure. They’re probably gonna find archeologic stuff from 5,000 years ago, if people knew that if you just cut the tubes that you could feel in the scrotum that led from the testicle to the prostate, and cut those tubes, you could stop the transfer of sperm and lead to a man becoming sterile. What we do in our office I think is very unique. The first thing that we offer is a single visit vasectomy. So in most clinics you’re going to have to go and spend an hour and a half to told exactly what you could read online about the vasectomy. There’s some benefit if you’re really afraid of what’s gonna happen or you want to meet the surgeon or you have a lot of questions or other urologic issues, there’s probably some value in having a consult first. But in our clinic, I will tell you that 95% of our patients really appreciate the convenience of a single visit vasectomy…
Donna Lee:
…because they’ve already kind of made up their mind.
Dr. Mistry:
They already know. And, it also, I think that a lot of men, you know, are a little afraid of having the procedure done. So by just kind of combining all the pain of having to go to the doctor in one little visit, I think is a real benefit. The second thing that we do is we offer IV sedation, which is with a sedative called Versed or Midazolam.
Donna Lee:
And from based on what the patients look like when they leave, it sure feels good.
Dr. Mistry:
They like it, you know, 90% of the time…
Donna Lee:
They are smiling.
Dr. Mistry:
…the joke is, of course, that, you know, “Can I take this home?” And what Midazolam does is it causes you not to remember, it completely relaxes the musculature of the lower abdomen so that your testicles fall lower, so there’s less pulling, because that pulling sensation is what bothers most men the most. To do IV sedation requires special authorization by the Texas State Medical Board and by the DEA. And, as far as I know, we’re, if not only, but one of the few people that does that.
Donna Lee:
How do other offices do a vasectomy?
Dr. Mistry:
Most of them will offer like a Valium. And if you take the valium beforehand, then you have to have been seen before. So if you get a consult before then you can use a Valium and then you can do the vasectomy the next day. But because we do a single visit, we can’t have you take Valium before you come in or else we can’t legally consent you for the procedure. Now you don’t have to have anything. You can just have come in and get local and about maybe 10% of our patients do that if they don’t have a ride or they don’t want to have to feel the after effects of a little sedative for the rest of the day. But I think that helps. I think it’s nice to go home and sleep because it probably cause you less discomfort.
Donna Lee:
Right, they’re always smiling. And one guy offered to buy a round of drinks for everybody when he was walking out.
Dr. Mistry:
I don’t think he knew it was not in a bar.
Donna Lee:
No, but that’s OK. We said, yes.
Dr. Mistry:
It was Friday.
Donna Lee:
We’re waiting.
Dr. Mistry:
We also do not use metal clips. Many urologists use metal clips because they’re quick. But in order to reduce the likelihood of postoperative complications and pain, we actually spend a lot of time taking all the tissue right around the vas deferens off and clearing it, and then we use a dissolvable stitch. We also use a dissolvable stitch to hide the two ends from one another to reduce the likelihood of the vasectomy auto-reversing, which is extraordinarily rare even though all of us seem to have a friend with whom this happened–you know, that one friend knows everybody I guess.
Donna Lee:
Oops.
Dr. Mistry:
So it’s the, we use the no needle, no scalpel technique. And here in our office we actually can do both sides with one incision. So a lot of times, 2 incisions are made. 1 incision leads to less healing time, less risk of infection, overall just a better cosmetic outcome. And this procedure has not been designed or implemented without significant thought. This isn’t just what I was taught in residency 15 years ago. This has been a procedure that I’ve been able to perfect and then implement across all of our providers. So we all do it the same, and you’re going to get the same experience and we’re very proud of offering that. Donna Lee, why don’t you tell people how to get ahold of us?
